Birth Certificates

Birth Certificate applications can be found here.

Certified copies of Birth Certificates:

  1. can be obtained for anyone born in the State of Ohio at our local office.
  2. are required for most official uses of vital records, such as school enrollment, driving permit/license, and marriage license.
  3. are printed on security paper and carry the seal of the State of Ohio Registrar.
  4. costs are $27.00. We accept cash, money order, and debit or credit cards. (Additional processing fee is required for debit or credit card purchases.)

How to obtain a birth certificate

Four (4) ways to request a birth certificate:

In-person:  The fastest way to obtain a birth certificate is by visiting the Health Department during business hours. We are located at 230 North Lincoln Ave, Salem Ohio 44460. (We are on the first floor of the Kent State City Center building near the handicap and 15-minute parking.) Applications are available in the office or online. Your request will be processed as you wait. Normal waiting time is 5 to 10 minutes.

By internet: For your convenience, you can process online requests through VitalChek LexisNexus– an independent company that the Salem City Health District has arranged with to provide you this service. VitalChek may be reached through their web site www.vitalchek.com. This option allows either overnight shipping or two day shipping via UPS (an additional fee of $7.00 + plus shipping is charged by VitalChek for this service). All major credit cards are accepted, including American Express, Discover, MasterCard or Visa.

By phone: To apply over the phone, call us during business hours at (330) 332-1618. You will be required to provide us with the information needed on the application, along with debit or credit card information. Birth certificates will be processed within 2 business days and shipped by mail. (Additional processing fee of $7.00 paid to VitalChek)

By mail: Applications may be filled out and sent by mail to the Health Department with payment. If you do not have access to a printer, the information from the application may be written on a piece of paper and mailed in along with a self-addressed stamped envelope.

For the person receiving the certificate, you will provide:

  • Name (First, Middle, Last)
  • Birthday (MM/DD/YYYY)
  • Father’s Name (First, Middle, Last)

Mother’s Maiden Name (First, Middle Last)

For the person applying for the certificate, you will provide:

  • Name (First, Middle, Last)
  • Address
  • Phone Number

*If you are applying for a birth certificate for yourself, you are both the recipient and the applicant.

You may send the application to the Salem City Health District, 230 N. Lincoln Ave., Suite 104, Salem, Ohio, 44460 with a payment of $27.00 in the form of cash or money order. Money orders should be made payable to the Salem City Health District. The birth certificate will be processed when the application is received.

*No request will be filled if the application is incomplete.

Death Certificates

Death certificate applications can be found here.

Certified Copies of Death Certificates are:

  • only available for persons who has passed within the city limits of Salem. If a person was born or lived within the city limits of Salem, but passed in another county, their certificate will only be available in that county.
  • required for most official uses of vital records.
  • printed on security paper and carry the seal of the State of Ohio Registrar.
  • $27.00, we accept cash, money order, and debit or credit cards. (Additional processing fee of $2.50 for debit or credit card purchases.)

Important Notice regarding Death Certificates:

ORC 3705.25(5) Requires that the social security number be redacted off of any death certificate for a person who has died within the last five years unless the person applying for the certificate can prove that they are either a, spouse or legal partner, natural or adopted child, grandchild, or great-grandchild, funeral home, veteran’s administration officer, law enforcement agency, executor or administrator of estate, etc.  For a full list of relationship and what qualities as acceptable proof please call (330)332-1618.

About Salem City Health District Vital Records

Vital records include: 

  • Birth Certificates
  • Death Certificates
  • Paternity Affidavits
  • Burial Permits

The Salem City Health District (SCHD) maintains records of births and deaths occurring within the city limits of Salem from December 20, 1908 to present.

Records from 1867 to December 19, 1908 and Marriage Certificates, are provided by Columbiana County Court House, Office of Probate Court, 105 South Market, Lisbon, Ohio 44432 or at the Ohio Department of Health, Bureau of Vital Statistics, PO Box 15098, Columbus, Ohio 43215-0098. Prior to 1867, Ohio law did not require records to be kept.

Genealogy Research: Searches are conducted as time allows. We accept requests through by mail and will return information requested as soon as time allows. No copies of documents are issued. If you require a legal document, a State-certified certificate will be issued at a cost of $27.00. Please note that processing of current documents will take precedence over research of genealogy documents. You can also contact The Ohio Historical Society.
